License:
Must have a valid license to practice in the state of employment. Must meet all the requirements to participate in and offer therapy services under the State Department of Human Services as outlined in your corresponding profession’s Practice Act.
Must have a valid driver’s license
Education and Certificates:
Must be a graduate of an accredited curriculum from a qualified institution.
Demonstrate knowledge and skill in current therapy practice.
Must have a valid CPR certification
